This evaluation form [PDF 32 KB] has been provided for your convenience. Should you wish to reproduce the questions onto a separate form, please be sure to answer all sections and questions in the order they appear. Evaluation reports should be typewritten and point form is encouraged where appropriate. Full completion of this Project Evaluation Report form by 2011 CEPGP grant recipients is mandatory within one year of receiving funds or by Friday, August 31, 2012.
